Check spam mail to make sure you have received the waiver link. It will be one email one waiver link per athlete (each athlete&#8217;s waiver link is unique).<\/li>\n<li>Once you receive the completed forms, click on the link in the waiver email for the athlete and complete the waiver on behalf of the athlete\/parent. For signature, please type the athlete\/parent&#8217;s name.<\/li>\n<li>Submit the completed online waiver and keep the hard copy of the waiver form for your reference.<\/li>\n<\/ol>\n<p><strong>Submission via Email<\/strong><\/p>\n<p>This is only for teams who experience technical difficulties in submitting the PRW forms via online registration system.<\/p>\n<ol>\n<li>Print out the blank waiver form available under Events &#8211; Docs, Reports &amp; Results &#8211; Documents Related to this Event<\/li>\n<li>Pass the blank form to your athletes\/parents to complete the form.<\/li>\n<li>Scan the completed forms and arrange them by alphabetical order of the Last Name in a SINGLE PDF FILE.<\/li>\n<li>Email the PDF file to the official email with the subject header: &lt;Account ID&gt; Waiver Forms.\u00a0<em>E.g. 12345 Waiver Forms<\/em><\/li>\n<\/ol>\n<h1>CONTACT<\/h1>\n<p>If you encounter a problem which the solution cannot be found in either the Registration Guide or <a href=\"https:\/\/aseactivesports.com\/events\/acic\/registration\/faqs\/\">FAQs<\/a>, please email us at events@aseactivesports.com with the following information:<\/p>\n<ol>\n<li>Your Account ID<\/li>\n<li>Description of the problem<\/li>\n<li>Screenshot(s) of the problem<\/li>\n<li>Details involved in the problem<br \/>\nE.g. athlete&#8217;s name, athlete&#8217;s birth date, team name, division, etc<\/li>\n<\/ol>\n<p>As our online registration system is based in the US, there is a 12-hour time difference. For problems which require the assistance of the system programmers, please exercise patience while we attend to your problem as fast as we can.
