Participants Release and Waiver Form
A blank copy of the Release and Waiver Form is available for download in the Online Registration System. To access it, login and go to COMPETITIONS – [Select Event] Go! – Docs, Reports & Results – [Under Documents Related to this Event] Blank Waiver Form
For companies who are processing the waiver forms on behalf of the athletes, you may download the blank waiver form, print it out and have the athletes/parents fill up the form. Assign a common email to the athletes’ ParentEmail and send the waivers . Once you have received the completed waiver form, please click on the link sent to the email to submit the waiver online on behalf of the athletes/parents. Please keep the hard copies of the waiver form for your reference.
Travel Package
We have provided a travel package which include accommodation and transportation (for airport and land transfer) option.
Our accommodation are limited and are reserved on first-come-first-served basis. Book early to avoid disappointment.
